Forms to complete prior to tournament:
1. Coach Survey - complete this with your team. One per team. We would like your feedback on what works, how to improve, and how to further inspire our students! Help us improve our program!
2. FIRST 2010/11 Consent Form - One form per team member, including coaches. Must be signed by legal guardian. You will turn these in at your Regional Tournament registration desk.
3. Team Demographics - complete one form per team. You will turn these in at your Regional Tournament registration desk.
